Portugal kampte lange tijd met een groot drugsprobleem. In 1999 was maar liefst 1 procent van de bevolking verslaafd aan heroïne. Het land had het hoogste aantal drugsgerelateerde Hiv-doden van Europa. In 2001 besloot Portugal alle drugsgebruik te decriminaliseren. Het bezit van kleine hoeveelheden drugs is sindsdien niet legaal, maar het is ook niet langer een strafbaar feit. Drugsgebruik wordt gezien als een gezondheidsprobleem. Drugsgebruikers worden sindsdien niet meer gearresteerd, maar moeten voor een speciale commissie verschijnen waar een dokter, een advocaat en een sociaal werker een behandeling voorschrijven of een boete geven. Het aantal heroïne-verslaafden is met 70 procent verminderd. Mede daardoor is het aantal diefstallen, aids-patiënten en drugsdoden sterk teruggelopen.
